A few thoughts on free surveying tools:
Google Docs is a good option. But the problem is that there is no way to prevent individuals from responding more than once, so it depends on what you want to achieve with the survey. If you want something that complies to EU data protection and privacy laws then Limeservice (a Germany-based option) might be worth investigating. Its FOSS too. https://www.limeservice.com/
If its just a strawpoll you're wanting to do, then Doodle is great. Its really quick to setup and has a nice simple interface. http://doodle.com/
Check the privacy policy for Qualitrics, as I did a quick Google and it looks like it might be the same as SurveyMonkey's.
If you want something very similar to SurveyMonkey, then SurveyGizmo might be a good option. Extra to SurveyMonkey it has open API access and also twitter integration. The data exporting options look quite good too. I've never used it, but it seems to be very well established. Its not free, but the sign up is quite low for professional use (providing you can subscribe on a monthly basis and leave whenever you like, you'd need to check this). http://www.surveygizmo.com/plans-pricing/ If you're looking at paid services, then QuestionPro is another option (US-based). http://www.questionpro.com/pricing/index.html
