To an extent.
Utilisation does matter when it comes to number of subnets (it is irrelevant for number of hosts), and whilst sites can get a /48 with no justification whatsoever, you do need to justify anything in excess of that. Robin could, I am sure, relate tales of what he went through for his /44.

They are regionally aggregated, but within a region /48s are allocated sequentially.
