According to the Cathopedia, s.v. San Vitale d'Assisi <http://it.cathopedia.org/wiki/San_Vitale_d%27Assisi>, after reposing since the late sixteenth century in Assisi's cathedral of San Rufino this Vitalis' remains were translated in 2001 to the church of San Vitale eremita (i.e., V. of Assisi) at Viole, a _frazione_ of today's Assisi. It's noted here <http://www.lavoce.it/articoli/20010608551.asp> that some of this V.'s relics are scattered but that those at Viole constitute a body that's practically complete.
V.'s church at Viole is located on the slopes of Mt. Subasio at what is thought to have been the site of the saint's own hermitage. An originally eighteenth-century structure, it was restored in 2001. An exterior view is here:
