I think we had similar symptoms which BB looked at.
The first step was to update to 9.1SP2 (SP3 was released yesterday)
TSM also commented "After logging into the test course, I noticed the "Module Page" was not checked as available in the Tools Availability Section. I enabled it by going to Control Panel -> Customisation -> Tool Availability -> Checked "Available" in the Content Area -> Submit. Now the Home Page (Module Page) is ever present from the student view (Edit Mode OFF)."

Not sure if anyone has experienced the same problem, however at UCB we are finding that when we archive some of our courses from our live Bb8 environment and then import them into our Bb9.1 test environment, the link to the course home page brings back the error "Broken Course Link: Item is Unavailable". This is what they students would see when they login and access the course. We have tried to change the course entry point thinking that this could be the solution, but to no avail. The only way to view the home page, is by accessing it through edit view.
Has anyone else experienced this problem, and if so can they offer any sort of solution.
