I would agree with what Gill and Daniel have said.
With the outcomes my mentor recommended that I use 2 columns - one for 'anticipated outcome' and one for 'actual outcome' as this allows you to say what you think you ought to learn and then reflect on what you have actually learnt and why they might be different. This method gives you lots of reflection which you can then cross-reference from your statement.
I think, with Chartership, you have to try and make it all about you (which I'm finding hard too!) so, while it's great that your colleagues can now use the LMS, your PPDP entry needs to be about what you've gained (training skills, more time for your other work now your colleagues know how to use the LMS), how/why you've gained it, are there any further training needs that have arisen because of it.
