What you need is two versions of the NHS Postcode file ... the latest and
one from, say, 2005. Each one of these maps postcodes to SOAs to PCTs. The
2005 version will have the older PCT codes. Linking these two files using a
relational database (Access or SQL Server) should give you what you want.
The ONS website has the NHS postcode files.
