There was a series of papers in JAMA in 1990 by David Eddy. In one of them, he made the obvious point that anyone who adheres to guidelines 100% of the time must be negligent.
I think the paper was: D. M. Eddy. Clinical decision making: from theory to practice. Designing a practice policy. Standards, guidelines, and options. JAMA 263 (22):3077, 3081, 3084, 1990.
