We use lockable CD and DVD cases that are supplied to us by Intellident. The "smartSaferT" lockable case replaces the original and opens and closes by swiping it through the "unlocker" mounted on a wall beyond our security perimeter. The case itself is tagged inside, rather than the metal discs.
The cases have a clear display on the catch that either shows a red pad lock (locked) on a green open padlock (unlocked). They are the same dimensions as a normal DVD Case, original cover inserts can be taken from the original box and slide it the new one very easily (the CD cases are more like Nintendo DS games boxes but can accommodate CD covers ok). The maximum number of discs is 4 per box - although I think we have put more in some! This means for box sets we have had to split collections, but no-one has complained.
When we first introduced them for our cinema collection, I had many sleepless nights worrying that I would come in and find all our DVDs missing and a pile of cracked cases in the toilets - but we have never had a problem with this system in the 3 years since we first started using it.
Previously we kept our discs behind desks and students had to request them, it took a long time to locate the DVDs, put them in cases, and issue them. So the system has been very popular with library staff.
All that said, the cases are not cheap - but we are of the opinion that the investment has been well worth it.

most heavily stolen DVDs) and for a small selection of more valuable
CDs.
Our cases were supplied by D-tech who also supplied our self-service
system.
Our experience over the last 8 months we've been using the system has
been reasonably positive - not too many broken or failing cases.
However very few of our items will yet have gone through multiple
lockings/unlockings so we don't know how long they will last. We have
found the DVD cases more sturdy than CD ones
