On this page of the ABC of Working With Schools there is a link to a really useful publication produced by Renaissance East of England Evaluation Toolkit for Museum Practitioners which may well have all that you need. Scroll down to the useful websites bit at the bottom.

Subject: evaluation
Hi Everyone,
I would like to put together an evaluation strategy for our Museum, e.g. why we do evaluation and how we use it and set up some systems for evaluation feeding back into our work. Does anyone else have a similar policy or framework I could take a peek at?
Thanks
Trudie
