For what its worth, my SAUSAGES approach would give eGFRs of
15 if female and 20 if male for a 292 umol/L 'white' creatinine on a
Roche compensated Jaffe.
By my reckoning the 'new 175' would give 14 and 19
and the 'old 186' 15 and 21 ml/min/1.73m2, respectively.
Is it simply a gender thing?

One of our renal unit nurses noted a significant discrepancy between the laboratory eGFR and that calculated by the Proton programme from our creatinine result.. The patient is a 58 year old female seen in the renal outpatient clinic, who had a serum creatinine of 292 umol/L :
Roche Modular Proton
eGFR ml/min/1.73m2 14 21
A similar difference was seen a month earlier. Usually results on renal patients are no more than 1.0 different. The renal consultant is inclined to believe the Proton result over ours.
The only point of note is that she is a Thai and possibly the Proton programme uses a different ethnicity factor for east Asian patients. However, I thought that the only factor validated so far was for Afro-Caribbean patients. If there was a factor for patients from east Asia isn't more likely to be less than 1.0?
Has anyone else come across this level of difference? Your thoughts and views would be most appreciated.
