Alison, I have placed all our competency sheets online. IT were helpful and it was done via a link through the HR "Assessments and appraisals for KSF information" page.
Each member of staff is responsible for their own competence sheet and it is to be kept in their personal portfolio. Either I or the company trainer will sign off the initial level 1 (performs under supervision) at training, and the ward manager signs off Level 2 (performs without supervision). There is a column for annual update.
If anyone would like a copy of the ones I have for Clinitek 50, Clinitek Status, Bayer Contour meter, Multistix 8SG, SureScreen DOA, IL GEM 4000 blood gas and OPL co-oximeter, Coaguchek Xs Pro or anything else I may have, I am very willing to share.
