At my previous hospital we reported hemoglobins from the blood gas
analysers ( Radiometer ). I found the agreement with the hematology lab to
be very good but there were occasional outliers giving significantly higher
values on the blood gas instrument.
I believe they were due to insufficient mixing of the sample. Hemoglobin
results are more sensitive to incomplete mixing than the pO2 readings , so
a short twirl sufficient for an accurate pO2 reading may not be sufficient
for a reliable hemoglobin.
