Standard practice to give thiamine to all at risk of Wernicke's
(although often only the first dose of prevention is given). Not much
emphasis on thiamine before glucose, as glucose containing solutions not
much used.
I think that the basic teaching points to the juinior doctors in an ED
of giving thiamine to the risk groups is probably much more important
than over-complicating by worrying about thiamine before glucose.
