Dear Jason:

We are using MySQL+PHP (version 4.3) in a Linux box.  We developed a 
custom  interface that is easy enough for any clinicians as well as 
research coordinators to enter data.  I use phpMyadmin when I want to 
export tables and etc. 

As for organizing data, we sort everything by two primary keys: case# 
and visit#.   This may not be very elegant approach but it's been 
working for our needs.
