Looking back at the original question...what's wanted is
"a survey to see how campus life is going for disabled students, staff, and
faculty".
I think it's going to be necessary to pin down exactly what it is that's
being looked for. It might be that a simplistic medical model definition
based on impairment rather than "disability" within the British social model
would be the most appropriate.

I would think the universalist approach might be best , that is to say
starting
out not by targetting any group of students but by asking all students what
barriers they have found in achieving what they set out to, or expected from
university, with a few prompts and reminders and test questions that ask in
a
different way.
All in all one wants a flawless statistical matrix of valid answers that the
best mathemetician cannot deconstruct by sleight of hand, never mind the
sociologists and wafflers.
