Good afternoon, my organisation is currently producing draft Standards
and Recommended Practices for Medical Records Management and we are
targeting the implementation of a Unified Health Record for each patient
in our Hospitals. At present many individual hospital departments hold
their own client records and it is anticipated that there may be a
reluctance to move away from this. E.g. Physio, Dietician etc. I would
be grateful for the opportunity to explore this subject with any
Hospital Medical Records people who may have already faced up to and
dealt with this issue.

Is this register a single item? if so then I don't think the auditor's
understand how records are maintained. This is like attorneys here in
the states calling it a document retention schedule, since they tend
to focus on individual documents rather than groups of records.
If the register is a single item that contains multiple entries, then
I would explain to the auditor that a new register is opened at the
beginning of the calendar year and is closed at the end of the
calendar years. Retention is applied to the whole register and its
contents and the retention clock doesn't start until the register is
closed. Explain that retention is not done on an entry by entry basis.

Hi I work for the United Kingdom Atomic Authority at Dounreay, Scotland
and
we treat Lab books as log books which are kept for min 100 years. They
are
A4 hard backed and filed by year and location of lab. For last few years
we
also have a "Lotus Notes" database for keep similar info but the
handwritten ones are the ones that the Regulators wish to see. We tried
scanning but could not destroy the hard copies so there was little
advantage in scanning

Nicola,
I have previously worked for organisations that use R&D laboratory
notebooks
(pharmaceutical industry) very similar to the ones you describe.
Scanning is typically done for two reasons. Firstly, for disaster
recovery
purposes i.e. to create an additional security copy. Secondly, to
provide
improved access to the content whilst maintaining an acceptable level of
security and control over the original record. Many organisations
perform
scanning in-house (or at least on site) because they are usually
reluctant
to release the books for an extended period of time due to the high
commercial value of the content and the need to have them always
accessible.
Those that scan off-site are usually those that wait until the book is
completely finished with, whilst those scanning on-site typically scan
at
defined intervals whilst the book is still in use.
Indexing is frequently a problem! Some organisations rely on a fairly
rudimentary index at the front of the book; this would include brief
details
of projects/products worked on across a series of pages. As Ian
suggests,
other organisations use some form of database for indexing purposes,
whilst
relying on the original record of course for patent/legal/regulatory
purposes. For a hand-written record, legibility will always be an issue
that
needs addressing. Most organisations attempt to minimise the issue by
using
a Standard Operating Procedure that requires writing to be legible. You
are,
of course, reliant on colleagues to be compliant with the SOP!! Some
organisations (few that I know of) permit the sticking in of
word-processed
text.
Regarding retention times, this would depend on what your lab notebooks
are
supporting. In my experience, the notebook would support a patent; the
retention time is therefore set to expiry of the patent. Having said
that,
the retention times in practice are typically longer as key notebooks
are
also part of the historical record of the company.

reasons. Firstly, for disaster recovery purposes i.e. to create an =

security copy. Secondly, to provide improved access to the content =

record. Many organisations perform scanning in-house (or at least on =
site) because
they are usually reluctant to release the books for an extended period =
of time
due to the high commercial value of the content and the need to have =
them
always accessible. Those that scan off-site are usually those that wait
=
until
the book is completely finished with, whilst those scanning on-site =
typically
scan at defined intervals whilst the book is still in =
use.<o:p></o:p></span></font></p>

organisations rely on a fairly rudimentary index at the front of the =
book; this
would include brief details of projects/products worked on across a =

pages. As Ian suggests, other organisations use some form of database =

indexing purposes, whilst relying on the original record of course for
patent/legal/regulatory purposes. For a hand-written record, legibility
=
will
always be an issue that needs addressing. Most organisations attempt to
minimise the issue by using a Standard Operating Procedure that requires
writing to be legible. You are, of course, reliant on colleagues to be
compliant with the SOP!! Some organisations (few that I know of) permit
=
the
sticking in of word-processed text.<o:p></o:p></span></font></p>

individuals as part of a similar exercise? Especially ones which help us

make records and information management 'fun'? Basic or complex content,

I'm also appealing for the best and worst of your RM case studies. I

we have various posts to the list and various sources for these, but

like to know what people use as their favourite examples. The worst=20
consequences of bad RM practices, and the organisations who have made
