Before a student is referred to me for support, the Disability Officer
(who is the Student Welfare Officer here) checks that the students have
completed the appropriate form. At the beginning of every subsequent
academic Year I tutor those students, I check whether they have
completed the form before supporting them during that academic year.
This prevents the situation you have described and which we have
experienced in the past.
