Adding to Ellen's thread - when I was a student nearly 20 years ago,
in the eastern part of Norway, my favorite midwifery wisdom gleaned
from the older midwives on the ward, was the concept that the active
phase of second stage begins when there is fetal hair on view at the
introitus, not retreating between contractions. Very rare to need
intervention for prolonged second stage if you start that clock during
crowning.
