Some of the examples come from health promotion services, and most can be
transferred into other settings.

I don't have any questionnaires to share with you, but I think that in
order to get a fuller picture of user needs it might be useful to use
other methods as well as a survey.
You could for example hold focus groups - in a group people would be
able to spark off ideas from each other, and debate the value of
different suggestions.
Or you could visit people in their own departments and get a feel for
the information needs they have during the course of their work, and
the resources that they have in their departments. This might also be a
way of you seeing service gaps and needs that they may be unaware of
themselves.
Forgive me if you have already planned to do this!
