There is a diagram of that sort in Selwyn Goldsmith's 'Universal Design', published by the Architectural Press, 2000.
It is called the Universal Design Pyramid and is Figure 1.1, on page 3 of the paperback edition.

Partnership Research: Involving service users in research design
Please would you bring the following website address to the attention of
researchers who may be interested in collaborative approaches to
research with
service users.

The website was produced as part of a project (funded by the ESRC
Research
Methods Programme - RES 333-25-0002) which also included training
activities
exploring research design and the development of partnership approaches
to
research with people with learning difficulties.
Although the website, which has been hosted by the University of
Manchester,
was
designed for use by both academic researchers and service user
researchers, we
had to comply with the centrally agreed University website format. This
means
that the site is not as accessible as we would wish. However, some
documents
(the 'Easy' versions) are much more accessible than others.
