An ED echo would certainly be helpful, and of course one would expect this
degree of shock to exhibit RAD and right heart strain on ECG. Rowley needs
to tell us if the decline was sudden and massive as might be more likely in
PE or was it a progressive downhill spiral and final collapse as might be
more in keeping with septic shock (though of course not exclusive).
BTW Rowley this has been very opportune as we discussed perjorative
management of the alcoholic at SHO teaching on Monday and my troops are
anxiously awaiting the next installment !
