In Shropshire we have tried 2 approaches via the 2 PCTs - in Telford
the local OOHs service (Shropdoc) placed Triage nurses to work alongside the
Emergency Dept Triage nurses to sieve out primary care cases. They found
that only 5% were suitable. In Shrewsbury a GP was sited in the Emergency
Department - primarily to meet GP access targets using an appointment system
but seeing any primary care cases that came in at the same time. Virtually
no Primary cars cases attended and his continuing presence could not be
justified.
We have a lot of emergency admissions coming into ED from 999 calls
and more than 1/2 emergency admissions to the hospital now come via the EDs.
