I have seen no replies to this so I hope you have had some off-list.
I take it that the 'standard' form you are using is the statutory one for private study and non-commercial research. No problem with that.
I trust that when you give permission for reproduction of material you hold you are either:
- giving approval for material in the copyright of the IEE; or
- giving approval on the basis that you hold the original, but that the user is responsible for obtaining any necessary permission from the copyright owner and is liable for any infringement which he might commit.
TNA does not use an application form, since applications vary so much. We issue a standard letter, adapted as necessary, giving permission for use. If you would like advice on reproduction rights and permissions, I suggest you contact Paul Johnson in TNA's Image Library, which runs that side of things here.

We're looking at updating our procedures for allowing reproductions of
Archive material. We have a standard copyright form for those
requesting copies for private use, but we ask researchers to contact us
again if they wish to reproduce the item (this includes images, text and
film).
Does anyone have a standard form for reproduction requests that they
would be willing to share? Any help would be welcome.
