There's also WuFoo (http://www.wufoo.com/), which is a generic
form-building service. We've used it for customer satisfaction surveys,
and it's really user-friendly. The only problem may be that I don't
think that they yet have the ability for you to integrate the form
easily into your own site (although the FAQ suggests it will happen).
