I see that Isabell has confirmed what I suspected. I can also tell you
that from my birth in 1950 to my younger daughter's in 1981 the system was
that a person's NHS number was the index 'number' on their birth
certificate which consists of the book identifier (all letters which, of
course, identify the local register office) and the serial number within
that book.
