This is addressed mainly to those working in the NHS, but input welcome
from anyone.
This PCT is looking at creating a single, multi-disciplinary care record
for children. We currently maintain separate files for each service that
has contact with the child - health visiting, district nursing,
occupational therapy, physiotherapy etc - but want to integrate these into
the medical file. The cultural and procedural challenges in doing this
seem to be mostly in access to the records by the different services,
physical organisation of the file, and keeping notes contemporaneous when
clinicians are visiting patients at home.
I'd really welcome thoughts on achieving this, and would particularly like
to hear from people in a primary care setting who have worked on similar
projects.
