I am not sure that this is the sort of thing you were looking for, but the
UK National Disability Team did a review of assessment tools and audits
that could be used to reflect on the provision offered for disabled
students at education institutions. You can find the report of this review

The tools/ audits that were selected for the report focus on inclusive
educational practice and meeting the legislative requirements of the UK
Disability Discrimination Act 1995 (particularly Part 4). The research
didn't look into social care assessments, but I thought I'd tell you about
this work as your original email suggested you might be interested in
assessments that were appropriate to other organisations.
