Hello,
Personally, I use a new consent form for each report I seek. As I will have explained to an employee why I seek a report, this ensures in my own mind as much as any other that the employee is "recently" aware of what is going on and one then doesn't get caught out when someone disputes the consent.

Subject: Consent for release of medical information
Hello All,
just a query. How long are the above forms valid for after an employee
has signed the consent form.
Regards
Julie
