SECOND REQUEST
Thank you for replies to messages posted in August on this topic. This is
a repeat request to anyone else who can send examples of hospital
discharge guides for older people.
SCIE has been sponsored by the Dept of Health to produce a concise and
user friendly guide that will address the particular issues that older
people may face after discharge. This project emanated from a request
from an older people’s advisory group that pointed to the lack of clear,
comprehensive and accessible information, especially that which addresses
planning for future care, adjusting lifestyles and understanding the
psychological changes that affect older people leaving hospital.
Hospital discharge is an area of huge importance for older people and the
experience of the process including assessment of needs, setting up of
required services and other arrangements can often be a negative one
fraught with difficulties and causing distress to individuals, their
families and carers. We hope that our proposed discharge guide would
provide information that would help individuals gain some sense of control
over their lives by guiding them along the recovery process as well as
signpost older people, their families and carers to valuable sources of
help and advice.
The Social Care Institute for Excellence places a high priority on
involving users of services and older people will play a central role in
the production of this guide.
