All this discussion re LVF & Amiodarone has brought up that hoary old question of the ALS guidelines for AF. Most ED docs I know think they are over enthusiastic with the use of electricity and try all other methods first (but not 300mg Amiodarone boluses I may add) unless the patient is quite compromised, which obviously includes the severe chest pain and hypotensives
Now I know that 'quite compromised' is hardly scientific nor objective but is it just me and a few like minded docs whos are doing this or is it more widespread..... and are there any moves a foot to update them in the next guidelines?
