I don't see anything wrong. I'm anal enough that I'd have used a real
form like "0." instead of just "0" for the values, but that's just me
instead of the standard.
I suspect you are in an area that doesn't get particularly heavy use,
perhaps explaining why it slipped past one vendor's testing... but it
does look to me like slipping past testing is exactly what has
happened.
