I think the LR idea may be a red herring, in that regardless, the data subject
should get a synopsis of the accesses to their record.
Personally, I believe I should have access to notes I wrote forever, but need
not have access to notes made by other people if the patient is not
registered with my Practice.
I'm quite unclear how the QOF handles patients who leave during the year.
