I can shed some light on these schemes dreamed up by PCTs in Devon.
At its most limited it involves practices collecting data on all referals
and sending it to the PCT.
As the local Trust is Foundation all referals are cost per case like
fundholding.
Next is a payment to examine referals much as we are used to doing with
prescribing.
The most radical is to offer practices 50% of any savings they make against
previous referals.
I think the point is noone will know they have not been refered!
The scheme in Torbay is slightly more ambitious and it involves GPs in
additional chronic disease monitoring (were that possible) and rewards them
for avoiding admns in the specified patients by pro-active care 9whatever
that is).
