I don't know Latours work, so I will be off to read this, however I did infer
there was a concern over the power dimensions, micro and indeed macro as i
would see it within the clinical micro and macrosystems. Neither can be
disconnected from the other as there is a diffusion between the micro and macro
dimensions. 

The issue is that alteration of the power within the different temporal spaces
within the systems (I see them as 3 dimensional) suggests that any change will
casue a shift and movement of power. The issue then arises  of how this happens
withou the potenital power space being 'owned' by another perspective. It is
the tension between the micr and macro dimensions that offer a degree of
equilibrium. I have no idea how that is achieved without consensus and many
many conversations...is there time for these?
