When, as in this situation, IV access (and indeed access to the patient
himself) is extremetly difficult - major injuries in 3 limbs including
pelvis - IM Ketamine to aid extrication is ideal in my opinion. Treatment
at scene is always a compromise. We continued with ketamine IV in the resus
room for log-rolling, manipulation & reduction and splinting of the
fractures - titrated intravenously.
