I would appreciate if you could please tell about the clinical markers used
to identify what the patient is explaining is spasticity

this is a very valid approach to the problem of grading spasticity, since
neither Ashworth nor the modified version seem to be very reliable.
Spasticity varies all the time depending on many factors like general
health, fatigue, pain, stress a s o ,so why not ask the patient about how
he/she experiences it? We are now trying to use the visual analogue scale
and at the bottom of the scale spasticity is no problem and at the top it is
a very severe one. That way we hope to get a more complete picture of the
actual effect that spasticity has in the patient`s life. Information
straight from the source, so to speak. If the patient can not communicate -
well, that is another matter, of course. Maybe the carer would be a good
alternative?
This ought to work if the point with measuring spasticity is to find a way
to help a particular patient since if spasticity isn´t a problem for
him/her, we do not have to deal with it - or am I completely wrong here?

MAS does not measure spasticity but resistance to passive movement.
