have had to leave their jobs for reasons of disability

of Conduct)
Thanks for taking the time to respond. You do not need to be either an
employer or an employee. You can respond to one or both online feedback
forms. You do not need to make a written response. Both forms are
basically the same, but employees and employers will be expected to follow
different codes. Anyone with an interest is entitled to have an input as to
what these codes might be. If you work through either form you will be
given the option to state your interest, e.g. service user.
Complete the form online and press the 'submit' button at the end. That's
all there is to it!
