I think Jenny's suggestion of using this question in a follow up
interview would be preferable.
I think that questions about application profiles at the questionnaire
stage would put people off filling it in.

that have application profiles, and the question could easily fit into
the survey just after the question "Are you using any extensions to IMS
MD?".

those who are already using metadata to respond to this question. We
need to keep the survey questionnaire fairly short to encourage people
to respond, and a question about application profiles might also imply
that the survey is targetted at those already using metadata.
What do others think? Should we ask about application profiles in the
survey questionnaire?
