My practice seems to be as for the majority of those feeding back so far - I
record managers comments as those made by the employee.
My reason for doing this is that in my view it gives a rounded view. From
the referral perspective the notes then reflect the reason for referral in
addition to the managers comments, some of which may not be in the
management referral but still have a significant emphasis on your
assessment.
From an assessment perspective the client's subjective and objective view
points often help to 'put the meat on the bones'.
As a OH practitioner having and recording both sets of information helps me
to reach an independent opinion. Otherwise it would be like having a book
with key chapters missing.
