It depends what sort of animals (I can only really talk about poultry),
if that is of any use.
Our respiratory surveillance is HSE defined intermediate level (although
we are reviewing going to high level surveillance)- this is due to the
massive bacterial loading in the poultry shed litter/excrement as well
as the XSive endotoxin levels found in poultry dust. (I'd be happy to
supply numbers if interested).
We conduct skin surveillance by questionnaire - risk of erysipelas and
staph infection (amongst others).
General health/GI upset questionnaires aim to identify anyone with
symptoms relating to zoonoses. There is an excellent guide to the
occupational zoonoses from HSE: ISBN 0 11 886397 5. The Canadian HSE has
an excellent site on bacterial agent M.S.D.S.'s

The self administered musculo-skeletal tick questionnaire (from Cornell
University) for identifying musculo-skeletal problems.
Of course if you're talking lab animals that's not my field!
