Dear Mike. I have seen your questionnaire. Besides the point about social or
medical model the questionnaire is  based on the assumption that the
student/applicant have a good understanding of what the institution should
do for him/her. From my experience this is no the case. I  would prefer a
close answer type of questionnaire that will give them some idea of what to
expect: For example: do you need a shower chair?, a non smoking
room/corridor ?, a special fire alarm? A question like what do you think you
need from us? expects too much (or too little, depending on the real
intention) and receives very litlle as a response. Hope this helps, AV/UKC
