There is an article in the current Canadian Journal on Aging on "Resources
and Costs Associated with Disabilities of Elderly People Living at Home and
in Institutions" vol 20 (1), 1-21.
There was an attempt at a new "living standards" method of making estimates,
and a very useful review (p.69-83) of previous methods (expenditure diaries,
asking about extra costs, household budget comparisons) in the UK by
Berthoud et al. "The economic problems of disabled people" (1993), still
available from www.psi.org.uk
The UK-based Centre for Research in Social Policy has published a report on
the costs of childhood disability:
