This may be a bit late, as a response to your first
question, but have you considered Innocent III's De sacro altaris
mysterio libri sex (PL 217, col. 763ff). It certainly mentions
vestments, etc. and may be of some interest, as well, for your
second question. The same applies to Pierre de Roissy's Manuale
de Mysteriis Ecclesiae, for which see Marie-Thérèse d'Alverny, 'Les
