Don't see p/f that often now but a couple of years back in S.Africa
where I was treating a lot of sports injuries I saw it often especially
in long distance runners.
Typically c/o pain on the antero/medial aspect of the calcaneus which
was usually worse first thing in the morning as they got out of bed and
eased slightly after a few minutes of walking around. If they had
moderate to severe p/f this pain could be reproduced by passive stretch
of the p/f. Otherwise palpation. Commonly they were excessive pronators,
often with tight calves/tendo-achilles complex.
Basic treatment consisted of a variety of the following
- avoidance/reduction of activity - with the use of ice post
activity (a good home treatment is to put your foot on a tin of frozen
peas and roll it under the arch of your foot for 6-10mins several times
a day
- biomechanical assessment often done in conjunction with an
orthotist /podiatrist - often required orthotics [usually to prevent
some form of excessive pronation]
- Heel cups and/ or strapping
- Stretching - p/f and long foot flexors, gastrocs, soleus etc etc
- Sometimes Deep T/v friction and ultrasound (US only in acute
stage)
- Night Splint if chronic
There was no magic recipe for this - each patient required a good
assessment (don't they always)!!
