I have managed one 24 year old with SVT in her first pregnancy.
She had a long history of palpitations that usually reverted to sinus rhythm
with rest .This time things had not settled as usual.
After unsuccessfully trying the valsalva maneuver I tried carotid sinus
massage.
This response was very much like that with adenosine - Patient felt a sudden
discomfort as her heart stopped for a few moments and her rate fell from 240
to <100 and stayed there. First time I've had such good results with carotid
sinus massage! My next option would have been adenosine which is reportedly
safe in pregnancy (BNF)
