The librarians in the South West of England revised the
original NLM scheme and have been working with their own
Wessex Scheme for many years. These are maintained by the
University of Southampton Healthcare Libraries Unit on
behalf of the South West Regional Library and Information
Network (SWRLIN).

The subject indexing is done using MeSH and there is a MeSH
linked subject index to the classification scheme that is
also on the web pages.
Of course, now that NLM 5th edition is available on the
web, you might consider this to be the easiest route.
There is a lot to be said for using NLM products as then
you don't have to do all of the continuing development and
revision yourself. I can confirm that developing your
own scheme can be incredibly time consuming - even though
it might be professionally challenging.
