I have encountered the same puzzling problems a number of times. I have been
unable to enter an ISBN in the 020 field while cataloguing some new items
because the same numbers already belonged to completely different items.
I am afraid I do not know the answer to Andy McDonald's question, although I
suspect that it all comes down to publishing errors.
I would be very interested in hearing other explanations.

Right, I'm new to this discussion list but I'm erm, "puzzled" over the

ISBN 0333903897 - according to local catalogues (Bournemouth Libraries) and
Talisbase, the ISBN belongs to a book called "Tenants and Trustees" by Paul
Weibe. However, according to an Askews preview list, it belongs to "The
River Folk" by Margaret Dickinson. The former was published in 1981, the
latter is brand new. Possibly the former has gone out of print.
Now, this may sound incredibly dumb, but do ISBNs get (for want of a better
word) recycled? e.g. a book that goes out of print has its ISBN reassigned
to a different title? It would explain a lot if that was the case. Or is it
more likely that the preview list got the IBN wrong?
