Without indicating an authority for the
assertion, Lord Birkenhead says (p. 277)
that RK was "responsible for choosing
the inscription set up in every war
cemetery: 'Their name liveth for
evermore.' "
This is not quite the same as saying
that it's Kipling's phrase. It sounds
rather biblical to me.
