It sounds as if you have done a McKenzie assessment on this lady. From what you have described I would not be surprised if she is getting a lot of her trouble from her spondylolisthesis and of course repeated exercises in these clients should not be done.Because she has varying symptoms in different positions I would also suspect that the nucleus is protruding mainly R postero laterally and being severely compromised when she flexes due to the spondylolisthesis.
I would suggest that she obtains as much relief as possible by using her flexion position, working really hard on her transverse abdominus and multifidious exercises. You can then gradually increase her extension exercises and walking.
You will probably need to look at some pain relief/nasaids as well.
Depending on the grade of the spondylolisthesis then take the extension exercises very slowly and if her leg symptoms increase then you must stop doing mckenzie exercises.
