Agree this is not a DP issue. The first use of the false reference sounds like a straightforward attempt to obtain a pecuniary advantage (the opportunity to earn remuneration or greater remuneration in an office or employment) by deception. The appropriate investigating authority would be the police (but don't tell them I said so). The police could then follow up on the second application if they felt there were grounds to do so.

Logic says to me that the sole thing to be done is to remind the NHS Fraud
Office of the original report together with the fact that this person is now
an NHS employee. This report may not allege any further impropriety since
none may have taken place.
To me this leaves the DP arena and comes much more into either an allegation
of fraud, or potentially an action from the student against someone for
alleged defamation.
It may be that the NHS Fraud Office keeps records of all reports, whether
action is taken or not, and has been contacted as a matter of course by the
employing organisation

Some help with the following scenario would be most welcome!
Recently a member of staff at the University became aware that a student had
written a fraudulent reference in the member of staff's name on NHS headed
notepaper (this was a clinical degree) which the student had used to try and
obtain a job. The member of staff informed the NHS fraud office of this but
to our knowledge no further action was taken as this wasn't an NHS member of
staff - no further action was taken by the University either.
The department have now contacted me to say that they have learnt that the
student has now been employed by the NHS locally on a project, and are
assuming that the people involved in hiring the student would not have done
so had they known about this case ...
Their question is can they / should they inform the relevant NHS department
about this case? They are worried that the fradulent reference may have been
used again ...
